Duties include: Developing Resident Care Plan based on admission social services history, resident’s health, psychological, & social needs; Visiting & interviewing residents at quarterly intervals & when requested by staff, physicians, residents, or family members to determine recreational/activity history, needs, & interests, as well as social services’ needs; Counseling & assisting families with issues or concerns relating to caring for resident; Developing measurable social service goals & revising goals & care plan quarterly; Attending resident care plan & resident evaluation committee meetings for residents in areas of responsibility; Completing documentation regarding social service needs as required by regulations; Coordinating, facilitating, & organizing educational programs for residents regarding health, safety, & quality of life concerns; Monitoring Medicare eligibility period & providing required notices to residents/responsible parties; Providing general case management (including intake) & referral services to all residents needing such assistance; Providing individual casework, case management for frail or at risk residents to include assessment, development of individual care plan, coordination of services, & monitoring; Serving as liaison with appropriate social & health agencies & service providers in community; Referring & linking residents of community to local service providers, such as case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, visiting nurse, preventive health screening/wellness, & legal advocacy; Educating residents & families on service availability, application procedures, client rights, etc., providing advocacy as appropriate; Monitoring ongoing provision of services from community agencies & keeping case management & provider agency current with progress of individual; Managing provision of supportive services where applicable; Reporting any suspected cases of abuse, neglect, or exploitation of resident to appropriate agency; Attending in-service training workshops & meetings as required; Performing other duties as assigned. Required qualifications: Bachelor's degree in social work or related programs such as gerontology, psychology, or recreation therapy. 2 yrs exp. preferred. Social services certification preferred. Full-time, Temp-to-Hire position. Wage: $25.00-$27.00 hr. (#4.0318.206 – San Luis Obispo)
