Needed in Fresno, CA. Will provide mental health treatment services to clients in accordance with their treatment plans. Duties include: Completing assessments of each client & developing a plan for mental health treatment; Coordinating with other STRTP & MHP providers to ensure services are accessible & appropriate for each youth; Monitoring essential areas of treatment plan including medication reviews by psychiatrist & transition planning; Providing mental health services including individual, group, & family therapy, case management, collateral, & EPSDT as applicable; Providing crisis intervention with de-escalation methods to clients as needed; Participating in clinical review sessions with each client & providing insight & feedback during clinical reviews with mental health team; Working closely with mental health rehabilitation specialist to ensure consistent & collaborative approached is implemented for each individual resident; Clinically assisting with delivery of care; Maintaining confidentiality of records relating to clients’ treatment; Preparing & maintaining all required treatment records & reports; Collaborating with other staff members to perform clinical assessments & develop treatment plans; Completing comprehensive mental health assessment of clients in STRTP within 5 calendar days of admission; Facilitating initial mental health treatment plan for each client within 10 calendar days of admission; Reviewing with client their course of treatment & preparing updated treatment plans when clinically appropriate & no less than every 90 days; Reviewing client’s progress in treatment every 30 days; Completing all necessary reports &/or documentation required for preparation of client reports & progress reports; Modifying treatment activities & approaches as needed in order to comply with changes in client’s status; Participating in clinical reviews with other mental health staff including Head of Service to review STRTP clients’ course of treatment & transition plans every 90 days; Meeting with families, probation officers, police, & other interested parties in order to exchange necessary information during treatment process; Monitoring client’s use of medications; Scheduling medication reviews for clients with mental health conditions who may or may not be taking psychotropic medication; Providing information to physician/psychiatrist about client’s medication patterns & side effects observed/reported; Guiding clients in development of skills & strategies for dealing with their problems; Counseling clients, individually & in group sessions, designed to provide reduction of mental disability & restoration, improvement, or maintenance of functioning; Referring patients, clients, or family members to community resources or specialist as needed; Counseling family members to assist them in understanding, dealing with, & supporting patients or clients; Evaluating effectiveness of counseling programs & clients’ progress in resolving identified problems & moving towards defined objectives; Gathering information about community mental health needs & resources that could be used in conjunction with treatment plan; Preparing with each client necessary plans for transition to lower level of care, permanency, or other treatment setting; Completing necessary activities to ensure viable plan for aftercare with STRTP follow-up directives; Maintaining responsibility of implementation of individual, group, & family therapy; Assisting Head of Service to ensure compliance with local, state, & federal regulatory requirements, including timely & accurate documentation of services provided; Assisting with family therapy with assigned clients & their families; Assisting with crisis coordination & corresponding clinical support, to include availability for 24/7 emergency response; Providing conflict de-escalation & mediation involving youth as needed; Attending client meetings; Applying reasonable & prudent parent standard as appropriate; Providing Intensive Care Coordination (ICC) as agency’s appointed Intensive Care Coordinator; Facilitating provision of services including access, planning, delivery, & transition in accordance with medical necessary criteria & treatment inputs; Ensuring participation by child or youth, family or caregiver & significant others, along with any other STRTP staff in assessment & treatment planning process; Participating in CFT meetings to assist efforts of collaboration with child welfare systems across agencies; Integrating other identified support person(s) in child’s life & appropriate staff into existing CFT; Monitoring youth’s progress in meeting TX plan goals relating to ICC; Maintaining documentation (progress notes) related to interventions used by ICC coordinator & other mental health clinicians in meeting youth’s treatment plan goals. Required qualifications: At least 21 yrs of age or older; Meet minimum requirements of one of the professional disciplines of Psychologist, Licensed Clinical Social Worker (LCSW), Licensed Marriage & Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC); Possess graduate degree or higher in Counselor Education; Valid license to practice as Psychologist, LCSW, LMFT, or LPCC as issued by American Psychology Association (APA) or CA Board of Behavioral Sciences (BBS) OR registered by APA or BBS to practice as Registered Psychologist, Registered Associate Clinical Social Worker (ASW), Registered Associate Marriage & Family Therapist (AMFT), or Registered Associate Professional Clinical Counselor (APCC); Knowledge of practice guidelines of CA Child Welfare Core Practice Model (CPM). All certifications & credentials shall be verified by administration prior to hire. Mon-Fri/9:00am-5:00pm. Wage: $35.00 hr. (#4.0215.111 – Fresno)
